Based on the beautiful kdm-theme from mchavezg (http://www.kde-look.org/content/show.php?content=38714)
Icons are from Nuvola theme, the wallpaper comes from http://www.kde-look.org/content/show.php?content=37891, thx!

... picture, but the icons are wrong. Try to find icons that will match the picture...
