Actually, under the Windows Decoration section you can just select "Use custom titlebar button positions". That will do the job.

duh morons window decoration -> buttons -> CUSTOM BUTTON POSITIONS this theme is useless sorry ;-)
I installed this decoration, but the buttons are on the right like normal, not OSX style.
That's odd, it works fine for me. hmm... what version of KDE & IceWM do you have? Others please let me know if you're having the same problem.
I have kde 3.1, but I don't know how to check my icwm version.
