'index.theme' is a bit degenerate:

[Icon Theme]
Name=cursor-theme
Example=left_ptr
Inherits=core
----
Has the file been updated lately?
1. go to kde control center
2. click on peripherals - select mouse tab
3. click on cursor theme tab
4. install new theme
5. locate your downloaded file
6. select your newly installed theme from the list
7. restart kde

Still running strong on KDE 4.9!
It would be nice, if you will think about working version for GNOME ;) This one just does not appear in list :(
Would be nice to make it scalable. Thees are to big for me.
After I unzip to /.icons the pointers still do not show up in the customize pointers screen in gnome...
Just DragAndDrop it into theme manager.

ok, that would work on KDE... And in Gnome?
unpack the tarball to ~/.icons (~ = your home directory) and then go the cursor selection in the gnome-control-center. This works for every desktop.
